Under the Rose - Intake Form

It is important that you fill out this form prior to your first session.  

All information will be kept safe and confidential. All information will be viewed in a compassionate and non-judgemental capacity. Please fill out as much detail as possible.

The block of 3 sessions is £300 or 6 £550. This includes Essence of Rose Tincture for use during the sessions and for you deepen with in between, which I will send to you in the prior to your first session if we are working online, so please ensure you fill out in plenty of time so I have your address. Please pay in full before the first session, unless we have already agreed to a payment plan.
